If you wish to request leave from school for your child please complete this form at least four weeks beforehand. All leave taken without authorisation over four days will result in a penalty notice.

If your child requires medicine during the school day, you will need to complete a form. You can download this form here or collect from the school office. Once completed please return to the school office.
